Contract Between:

Parys Gymnastic Club  and Undersigned:

• Fees are payable before the 2nd of each new month. 
• I realise that interest will be charged on over due fees.
• I accept liability to give one month written notice (per email), should I or my child wish to stop classes.
• I understand by not paying the class fees, I will be held responsible for all over due fees, as well as interest, and if necessary, to pay all attorney and client costs, as well as collection commission, tracing fees and interest on over due accounts.
• Unfortunately at the event of overdue payment, the gymnast won’t be able to participate in training.

Indemnity:

I hereby waive and/or abandon any claim of whatsoever nature which I and/or the gymnast may have against Parys Gymnastic Club as a result of the participation in gymnastics and any other related activity or the utilisation of any of Parys Gymnastic Club facility or equipment.

I furthermore hereby indemnify and hold harmless Parys Gymnastic Club, from any damage of whatsoever nature (whether negligent or not), which shall include but not be limited to non-consequential damages, which I and/or the gymnast may incur as a result of the participation in gymnastics and any other related activity or the utilisation of any of Parys Gymnastic Club’s facility or equipment.

In the event of any accident and/or emergency, I hereby irrevocably and unconditionally authorise that I and/or the gymnast may be transported to the nearest medical facility for treatment. All costs occasioned by the transportation and/or treatment shall be for my own account and accordingly I hereby indemnifying hold harmless Parys Gymnastic Club it. Should Parys Gymnastic Club incur any costs in this regard, I undertake to immediately upon presentation thereof to refund such costs.

I recognise that participation in gymnastics and any other related activity or the utilisation of equipment may have inherent dangers which cannot be minimised or eliminated by any amount of key, caution, instruction or expertise and can result in severe injuries, which includes but is not limited to permanent paralysis, brain damage or death.

I am aware that photos and videos are taken from time to time by Parys Gymnastic Club for marketing and instructional purposes and I hereby consent that Parys Gymnastic Club may use any photograph/s or video/s of myself and/or the gymnast which was taken by Parys Gymnastic Club. 

I have read the entire indemnity and contract, and understand all the terms and conditions contained herein. I hereby confirm that I enter into this agreement will fully and without any undue influence.
